California medical mileage reimbursement

If you're treating for a work injury in California, the insurance company must reimburse you for the cost of getting to and from medical care — and most injured workers never claim it. Over months of appointments, that adds up to real money.

Under Labor Code §4600, you can be reimbursed for mileage to and from:

  • Medical appointments and physical therapy,
  • The pharmacy to pick up prescriptions,
  • QME and AME medical-legal evaluations.

The 2026 rate is 72.5¢ per mile for travel on or after January 1, 2026, regardless of your injury date. The rate is set by your travel date, so older trips are reimbursed at that year's rate (for example, 70¢ in 2025). You can also claim parking, tolls, and public-transit costs as separate line items.

The key is good records. Build a dated log of every trip — date, round-trip miles, and destination — and submit it with the DWC mileage form. This calculator creates exactly that log; enter your trips, then email or print a copy to submit. It's a small, simple win that's entirely yours to claim.

Frequently asked questions

What is the California medical mileage rate for 2026?
72.5 cents per mile for travel on or after January 1, 2026, regardless of your date of injury. The rate is based on when you traveled, so prior-year trips use that year's rate.
What trips can I claim mileage for?
Round trips to medical appointments, physical therapy, the pharmacy for prescriptions, and QME/AME evaluations related to your work injury. Parking, tolls, and transit fares are also reimbursable.
How do I get reimbursed for mileage?
Keep a dated log with the miles and destination of each trip and submit it to the claims administrator, usually with the DWC mileage form. This tool builds a printable log you can submit.
Is mileage reimbursed at the injury-date rate or the travel-date rate?
The travel-date rate. Each trip is reimbursed at the per-mile rate in effect when you actually traveled, not when you were injured.
